Care Coordination

A clearer path through your care.

When you need investigations, specialist care or better support at home, knowing what to do next can be difficult. Douvan provides doctor-led care coordination to help you navigate public and private healthcare in Jamaica, work towards a diagnosis and access appropriate treatment with fewer avoidable delays.

The 30-minute consultation with a Douvan doctor is free. We confirm the scope, availability and expected fee before you proceed.

Support where you need it.

  • Understand the next stepTests, referrals and treatment
  • Review care at homeNeeds, support and practical gaps
  • Keep care connectedAgreed medical oversight

You can come to us with a new concern, an abnormal result, an existing diagnosis or questions about care at home. You do not need to have had a Douvan home visit first.

From concern to diagnosis and treatment

We review your symptoms, medical history and available results, assess what needs attention and help plan the next steps. Where further care is needed, we explain suitable public and private options, taking account of clinical urgency, availability, your circumstances and the likely costs.

  • Coordinate appropriate tests, imaging, specialist review and facility-based care.
  • Help organise referrals and share relevant clinical information with your consent.
  • Follow up outstanding results and appointments, and help you understand the plan.
  • Stay involved through agreed follow-up as investigations and treatment progress.

The aim is to reduce avoidable delays. Appointment availability, diagnostic timelines and treatment decisions depend on the clinical situation and the providers involved.

Home-care assessment and planning

We assess the patient alongside the arrangements in place at home, identify what is working and what needs attention, and recommend practical changes.

  • Medical and functional needs, medicines and monitoring.
  • Caregiver responsibilities, support and ability to carry out the care plan.
  • Equipment, mobility and relevant safety concerns.
  • Gaps that may require nursing, rehabilitation, additional medical input or care in another setting.

You receive a clear summary of the priorities and recommended next steps.

Ongoing medical oversight

Where agreed, we provide ongoing medical oversight alongside your existing home-care team. This can include carers or nurses you already employ.

  • Planned medical reviews and review of treatment and medicines.
  • An agreed monitoring and follow-up plan.
  • Communication with carers, nurses and other treating clinicians, with consent.
  • Guidance on changes that need medical review and when care should be escalated.

We agree the scope, review frequency, communication arrangements and fees before ongoing care begins. Each independent provider remains responsible for the care they deliver.

Questions about coordinating care?

Do I need a home visit first?

No. Tell us what you need help with. We will advise whether the next step is a home assessment, a suitable consultation or another care arrangement.

Can you work with our existing carers or doctors?

Yes. With the appropriate consent, we can work alongside existing carers, nurses and treating clinicians. We agree our medical role and how communication will work.

Can I arrange this for a relative in Jamaica?

Yes. We can discuss the request with you and involve designated family members with the patient’s consent or appropriate legal authority.

How are arrangements and fees agreed?

We confirm the scope, availability and expected Douvan fee before you proceed. Where independent providers are involved, their services, availability and fees are confirmed separately.
